By registering my child for MTNVW Kids Parents Night Out, I acknowledge and agree to the following:

I give permission for my child(ren) to participate in all activities associated with this event, including games, crafts, group activities, and supervised play. I understand that reasonable precautions will be taken to ensure the safety and well-being of all participants.

I acknowledge that participation in children’s activities involves inherent risks (such as minor injuries from play), and I agree that MTNVW Kids, its staff, volunteers, or the church are not liable for minor injuries or incidents that may occur during normal participation.

In the event of a medical emergency, I authorize MTNVW Kids staff or volunteers to obtain necessary medical treatment for my child, including contacting emergency services if needed. I understand that I will be contacted immediately in such a situation.

I confirm that the information provided on this form is accurate, including any medical conditions, allergies, or special needs that may affect my child’s participation.

I understand that if my child becomes ill, injured, or unable to participate safely, I may be contacted and asked to pick them up early.
